Additional Details Trauma Level Charting System Cerner Unit Notes - Unit : PT, OT, Speech - Marian & Arroyo Grande (home based care) - Certs : Speech - do they prefer / require CCC-SLP and / or CCC-A credential? BLS required - EMR : Cerner - Rehab Aides / OTA / PTA : yes - usually will be perm staff though - Weekend Requirement : every 3rd weekend - Holidays : minor holidays, major only if needed - Scrub Color / Uniform : business attire with white lab coat, no jeans - Outpatient and Inpatient : N / A - Shifts : flexible, majority of work hours are M-F 8am-5pm - What areas are they required to cover : they will be assigned to a geographical region - Floor specific orientation : 1.5-2 weeks - Common diagnosis / Types of patients : PT / OT - ortho, strokes, ortho, joints, spinal injuries speech - anything - Additional Notes : Candidates must have experience with Oasis (gov't mandated form). Must also have one year experience working PT / OT / Sp, home setting experience is preferred but not required.

Physical Therapist, Home Health

Experience : 2 years

PT-CA(Copy needed)

Certifications : BLS-AHA(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 30 days of gap in entire work history

References : 1 Reference in the last 12 months

Must have experience with Oasis (gov't mandated form)
