About the Role
Join Corus Home Health in Albuquerque, where you'll deliver skilled, compassionate care that helps patients stay safe and independent at home. As a Registered Nurse on our close-knit team, you'll enjoy the autonomy of fieldwork, the support of a collaborative clinical team, and the opportunity to grow in a mission-driven organization.
Key Responsibilities
- Perform initial and ongoing assessments to develop and update personalized care plans
- Deliver skilled nursing services and specialized procedures per physician orders
- Complete timely, accurate documentation including OASIS assessments
- Educate patients and families on safety, medications, and health conditions
- Communicate with physicians regarding changes in patient condition
- Supervise and support LPNs and HHAs, including conducting supervisory visits
- Coordinate care with the interdisciplinary team and participate in team conferences
- Ensure compliance with all clinical, regulatory, and documentation standards
