Home Health Nurse

Job Description

Pride Health is currently seeking a Home Health RN for our client in North Charleston! This is a 3-month contracted opportunity with the possibility of extension. In this role, you will provide skilled nursing care in patients homes, coordinate care with an interdisciplinary team, and support patients' and families with dignity, compassion, and excellence.

Shift : M-F, on call required if requested

Pay : $39-$43 hourly, mileage reimbursed

Contract Length : 3+ months

Travel Area : Primarily upper Dorchester County, with coverage in Berkeley & Charleston Counties

What You'll Do

• Deliver skilled nursing care to patients of all ages in their homes.
• Assess patient needs in stable and emergent situations and develop / update individualized Plans of Care (POC).
• Provide patient and family education on care, diet, medications, and home safety.
• Ensure accurate, timely documentation using Homecare Homebase (HCHB) and OASIS.
• Supervise HHAs / LPNs to ensure continuity and quality of care.
• Monitor cases for compliance with payer requirements and arrange referrals when needed.
• Participate in interdisciplinary team meetings and provide on-call support as assigned.
