Washington State Licensed Health Coach

A company is looking for a Health Coach to support members with chronic conditions in achieving optimal health.

Key Responsibilities

Provide disease-specific care management interventions and promote self-management education

Assess members' needs and coordinate access to appropriate care and treatment

Document member interactions and develop individualized plans of care in collaboration with healthcare teams

Required Qualifications, Training, and Education

Bachelor's degree in a relevant field or equivalent experience required

Current, unrestricted license in Washington as LPN, RN, RD, or other relevant licensure preferred

Relevant certification such as Health Coach or Certified Diabetes Care and Education Specialist (CDCES) preferred

Minimum of two years of experience in disease management, diabetes education, or health coaching preferred

Experience in Managed Care, Medicare, and / or Medicaid preferred
