Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager.

Recruitment Requirements :

• High School diploma or equivalent preferred
• Ability to read and understand written instructions
• Ability to take direction via telephone in patient's home
• Ability to make meaningful observations and report changes in patient condition
• Basic knowledge of computer applications and ability to enter patient information into an electronic device
• Successfully pass all competency exams
