A company is looking for a Nurse Practitioner or Physician Assistant to join their Primary Care team.

Key Responsibilities :

Provide virtual care for a panel of patients (18+) through phone, messaging, and video

Ensure continuity of care for patients with varying health conditions

Collaborate with the care team on complex cases and follow up on labs and imaging

Required Qualifications :

DNP, FNP, ENP, or PA from an accredited program

Board Certification (NCCPA or AANP or ANCC)

Fluency in Spanish (read, write, and speak)

3+ years of in-person primary care experience

2+ years of experience delivering virtual care
