What You’ll Do

Position Summary

The Neighborhood Center Physician serves as a health-care professional who drives, supports, diagnose, and cares for a variety of health-related problems for Suvida Healthcare’s patients. The provider will manage these patients in a multidisciplinary value-based care model. The physician work assignments involve low-emerging risk complex patients to high complexity patients where the analysis of situations or data requires an in-depth evaluation of variable factors. The clinician will collaborate with a multidisciplinary care team and will serve as the leader of their respective care team within their Neighborhood Center.  Essential responsibilities consist of but are not all inclusive :
