The Program Coordinator is responsible for the coordination of client and family services . The Program Coordinator will support various agency programs and may include social services mental health educational and prevention programs . Alternative Family Services is a Foster Family Adoption and Mental Health Agency that has been serving abused and neglected children in our community since 1973. Our mission is to support vulnerable children and families in need of stability safety and well-being in communities. We provide a wide range of foster adoption mental health and transition-aged youth services in English and Spanish. We have eight locations throughout the Bay Area and over 200 employees. Driving Requirements
This position requires the employee to work out in the community and within foster homes throughout the Bay Area. Thus position requires employee to possess a valid CA drivers license and valid automobile insurance with his / her name on the verification card as an insured driver on the policy at all times. Position requires daily access to a car in safe working order.
Qualifications
- High school graduate or GEDrequired
- Minimum 3yearsexperiencein a socialservicerelatedfieldproviding services to children or families with complex needs
- Obtain andmaintainfingerprint and governmentand Agencyrequiredbackground clearances
- CPR / First Aid certification
- Able to driveand transport childreninto a variety of community settings using a personalvehicle that ismaintainedin safe working order
- Current and valid California drivers license with an acceptable driving record current and valid auto insurance showing as an insured driver on the policy and vehicle registration
- Possessa high levelof integrityhonestyand strong work ethic
- Able to read and write fluently in English

- Possess strong verbal and written communication skills
- Able to lead thefacilitationof group meetings
- Able to apply common sense understanding to carry out instructionsfurnishedin written oral or diagram form

Responsibilities
Conduct outreach and engagement activities tofacilitatelinkage to the services for various agency programs. Outreach and Engagementmayconsistof phone calls mailed information and field visitsServe as the day-to-day contact forclients and cases assignedto address questions about the status of referrals or cases assignedCoordinate with internal and external services for AFS clientsSupervision and oversight of program (ECM ISFC etc.) casesCollaboration withAFSQM departmentOffer services where the client lives seeks care or finds most easily accessible including office-based telehealth or field-based servicesLiaison with countyand community representativestoestablishand buildopen communicationsand relationshipsLearnand applyvarious evidence-based practices based on thepopulationsneedsto support referralsclientsand familiesContribute to a positive team cultureLeadandparticipateingroup programmingstaff meetings supportgroupsand training sessionsProvide services via methods consistent with the agencys standards and policiesAttention to detail tomonitorclient assignment and complete tasks in line with required timelinesAbility to engage clients in multiple modalities including text emailvirtualandin-personEscalate issues appropriately and provide input into continuous quality improvement effortsReview and approve documentation for quality of completion and timeliness ensuring all written material and computer data entry is completedin a timely mannerAttendandparticipateinmeetings as assignedMaintain client charts to ensure compliance withContractual / Medi-Cal / county / state regulationsVisit client sites and transport clients using personal vehicle that ismaintainedin safe working order into a variety of community settingsProvide emergency and crisisresponse.
