PEER SPECIALIST

Description:

DUTIES:

1. Provides outreach, consultation, education and prevention services to persons and communities as assigned by the Team Leader.
2. Provides information, education and support to family members and collaterals with the written informed consent of the client; Collaborates with other professional staff for metal health assessment and treatment for clients with dual diagnosis.
3. Develops, monitoring, implements, & revising in collaboration with client(s), an individualized person centered treatment plan base on the assessment including: patients problems, specific and measurable short and long range goals and specify the treatment intervention with a multidisciplinary treat.
4. Make contacts with new or potential referral sources in the community for services we provide.
5. Enters progress notes into the patient’s record documenting the content of contact with the patient or with collateral sources for the patient. Progress notes shall be filed in the medical record of the client within 24-48 hrs and include patient progress as related to the treatment plan and be signed and dated producing a continuous and unbroken record;
6. Documents transfer or change made in the patient’s level of care and includes the basis for the recommendation using approved placement criteria, date of transfer and name of the facility to which the client is transferred.
7. Multiple contacts with consumers may be as frequent as 2 or 3 times per day, seven days per week and depend on individual need and a mutually agreed upon plan between individuals and program staff. Many, if not all staff shall share responsibility for addressing the needs or all individuals requiring frequent contact. An average of 3 contacts per week for all consumers.
8. 80% of services must be face-to-face with consumer & 75% of services contacts must be in the community in a non-office based or non-facility based setting.
9. Participates in daily staff meetings & supervisory meetings with QP.
10. Assurance of Client Confidentiality, Rights, and Safety.
11. Knowledge of agency Policies & Procedures.
12. Keep all trainings required by the State and local agencies.
13. Perform other duties as assigned by Administrative Team
14. Work as a team player with co-workers & community agencies
15. Coordinate referrals, admissions, and review of consumers..
16. Transport consumers in company vehicle off-campus. (Valid Drivers License if your licenses is no longer valid you must immediately inform the CEO)

Requirements:

QUALIFICATIONS:

Minimum high school diploma or GED, or four-year degree in human service

Have life experiences in mental health, substances abuse, or mental illness.

Must be a NC Certified Peer Support Specialist.

Able to read, write, and understand directives.

Has no known history of abuse, neglect, or exploitation.
