We provide a broad range of medical and surgical services including minimally invasive and outpatient options. Our family of services includes the areas only inpatient oncology unit the Orthopedic Spine and Hand Center Family Birth Center the Lactation Support Center and retail store and many others. Job Summary and Responsibilities

As our Community Health Worker you will be a vital link between individuals and essential community resources actively working to reduce health disparities and improve access to support services.

Every day you will proactively engage with clients assessing their needs and identifying pathways to care and support. Youll connect individuals to relevant health social educational and employment resources providing guidance and practical assistance to overcome logistical barriers.

To be successful in this role you will possess a profound understanding of diverse community needs exceptional problem-solving abilities empathy and resilience. Your dedication to empowering clients and removing barriers to care will be paramount.

• Building community relationships and increasing program visibility within the community.
• Providing education on various health topics
• Enrolling participants in classes and workshops
• Completing and submitting all required documents and reports to Supervisor
• Clerical duties such as data entry making phone calls to participants and preparing workshop packets.
