CHI Health now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health includes 150 clinics delivering quality care to patients across Nebraska and southeast Iowa. Our full-service network provides a variety of Primary Care & Priority Care Services including Family Internal Geriatric and Pediatric Medicine in addition to several specialties to deliver custom care based on the unique needs of our patients.

This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patients experience and prep when they visit any of our clinics. We couldnt do it without you! Job Summary and Responsibilities

• Collaborate with providers to develop and implement care plans for select patient populations.
• Function as an interdisciplinary team liaison for patients by assessing securing and coordinating appropriate services through the continuum of care.
• Educate patients about diseases treatments side effects side effect symptom managementand how to access reliable sources of information.
• Answer questions as appropriate and serve as a liaison to the physician for additional questions and concerns.
• Identify and connect patients with community resources and support services including financial assistance programs and transportation services.
• Offer emotional support to help patients cope with the challenges of living with these diagnoses.
• Promote awareness of appropriate clinical trials in consultations with the provider.
• Other duties as assigned by management.
