Horizons Care Center is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization and

Hourly Pay : $33.38 - $37.82 per hour based on experience. Plus, earn $2.75 per hour shift differential on nights (6PM - 6AM)!

Schedule : PRN. Schedule days and times will vary. Occasional weekends required.

The LPN role shall perform in accordance with state and local laws and within the guidelines of his / her professional organization. Our person-centered care approach provides a low nurse to patient ratio, which allows you to make a real difference in the lives of those in your community. You will truly provide exceptional cares and clinical outcomes to your residents.

Required Qualifications :

Essentials :

Role models positive communication skills with NA / RS, TMA’s (if applicable), interdisciplinary team, customers, residents and families.

Maintains competence in all required skills within the facility.

Maintains all required documentation in an accurate and timely manner according to policies and procedures of the facility, including documentation of medications, treatments, restraints, psychotropic medications and skin condition.

Attend nurses meetings as scheduled.

Serve on, participate in and attend various committees and meetings of the facility to enhance interdisciplinary process.
