Case Manager 40 hours a week, M-F Client is looking for someone temp-to-perm, so LOCAL candidate is ideal. however, they will consider a 13 week travel assignment, too. MUST HAVE STRONG Inpatient Hospital Case Management experience BSN preferred but not required experience in Acute care Setting, plus experience in discharge. - Utilization review not required. Must be familiar with Medicare, Medicaide, manager care and other 3rd party payor review requirements Ensures medical necessity of hospitalization and treatment in accordance with hospital and third party payor guidelines and criteria. Ensures appropriate utilization of hospital resources. Works with other physicians and health care team members to manage care in a cost-effective and efficient manner. Responsible for the integration, coordination, and advocacy for patients and families requiring post-hospital services. Achieves planned care outcomes by planning and arranging services on discharge. Acts as liaison with Medical Staff, other departments, patient and family members and community resources to facilitate the utilization management review / discharge planning process
