Patients per day : 14 outpatients, no hospital

Info of support staff : 2 APPS IN OFFICE 1FT Physician .5 locums in clinic third week is tele medicine lab on site and infusion suite

Current EHR in use : Cerner

Submission Requirements

ALL MUST BE NOTATED AT TIME OF NAME CLEAR

• Board Certified (No BE) - Required
• MI License & CSR at Submission - Required at time of clear, no exceptions - not accepting IMLC
• BLS - Required
• Provider must be local - Highly preferred
• Must have clean background for quick credentialing- REQUIRED no exceptions

• DEA - Verbal Confirmation Required at Name Clear
