Position Summary
The Case Manager monitors service quality across assigned homes, ensuring client needs are met while fostering strong relationships with providers, clients, families, and community partners. This role requires flexibility, fieldwork, and a proactive approach to problem-solving, documentation, and service coordination.
Essential Duties & Responsibilities
- Manage a caseload of up to 20 clients, ensuring quality services are provided.
- Conduct weekly, bi-weekly, or monthly home visits based on assigned tier level.
- Complete a minimum of two unannounced visits per year for each client.
- Monitor client progress, maintain accurate and current records, and complete required reports (including quarterly reports) on time.
- Communicate promptly with providers, families, clients, and community agencies to ensure timely and effective service delivery.
- Collaborate with funding agencies, legal entities, medical professionals, and other community partners.
- Assess client behaviors and needs using professional expertise in psychology and social services.
- Participate in interdisciplinary team (IDT) meetings and client service planning.
- Provide transportation for clients when needed, sometimes on short notice.
- Assist with crisis intervention and stabilize services when issues arise.
- Coordinate client transitions and manage intake, referral, and assessment processes.
- Support program activities and community events, including event planning as needed.
- Adhere to all company policies, procedures, and regulatory requirements.
- Other duties as assigned.
Qualifications
Bachelors degree in Human Services or related field.Minimum two years of related experience.Valid drivers license, reliable vehicle, insurance, and registration.Ability to pass background check, drug screening, and fingerprint clearance.Flexible schedule, including on-call rotation and ability to work at various sites.Experience in crisis management and providing community-based services.Proficient in Microsoft Office (Word, Excel) and basic technology use.Strong written communication and documentation skills.Ability to maintain confidentiality and handle sensitive information appropriately.Familiarity with Title 17 regulations preferred.Physical & Work Environment Requirements
Ability to lift up to 50 lbs. occasionally.Frequent driving and travel to multiple locations in a workday.Ability to sit, stand, bend, stretch, and move between office and client environments.Clear vision, hearing, and verbal communication abilities required.Capacity to cope with family-related emotional situations and diverse lifestyles.Core Competencies
