Responsibilities and Duties

• Work with a registered nurse and other clinicians to provide care based on an individualized plan of care.
• Record your patient’s health status and report it to the nurse.
• Provide companionship and personalized care while encouraging independence and self-care.
• Assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
• Provide mobility and transfer assistance, which may include using mechanical lift devices.
• Prepare meals and snacks. May include grocery shopping.
• Perform light housekeeping, including vacuuming, laundry, and linen changes.
• Assist with incontinence care.
• Assist with transportation needs.

Qualifications and Skills

• Current and valid Home Health Aide certification
• CPR certification
• Ability to lift at least 50 pounds
• Availability to work on weekends, evenings, and holidays
• Reliable transportation
• Valid driver’s license and insurance
• Prior home health experience preferred
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
