Qualifications for Social Services :

LSW licensure required; LCSW or LICSW strongly preferred

Excellent communication, documentation, and assessment skills

Possess a genuine concern for and interest in people

A minimum 2 years’ experience working in a SNF with short

term rehab / discharge planning experience

Responsibilities for Social Services :

Build relationships between residents and staff to support resident’s individual needs

Find options that most meet the physical and emotional needs of each resident

Maintain contact with family to report changes in health, current goals, and care planning

Make referrals and obtain services from outside entities

Provide or arrange needed counseling services

Assist residents with financial and legal matters

Review facility policies and procedures as part of the facility's interdisciplinary team to assure compliance with state and federal regulations
