Job Description
A Community Health Worker (CHW) is a frontline healthcare professional who works within their community to promote health, wellness, and access to healthcare services.
Responsibilities:
- Support chronic disease management
- Provide health education and outreach
- Assist with navigation of healthcare systems
- Advocate for community health needs
- Connect individuals to social services
- Conduct health screenings and assessments
Qualifications:
- Typically reside in the community they serve
- Have a deep understanding of local culture and social issues
- Possess strong interpersonal and communication skills
- May have formal training in healthcare or community health, but not always
Role in Healthcare:

All applicants must complete a BCI check and haver own transportation and a clean driving record.
