The Role : The Patient Support Associate supports the ongoing care and retention of Rippl patients who are already active in care. This role is responsible for following up on fall-offs, assisting with rescheduling, managing ongoing documentation and chart hygiene, and serving as a key operational support partner to the clinical care team. The goal is to ensure patients continue to receive consistent, uninterrupted care and feel supported throughout their journey.

Essential Functions :

• Proactively reach out to patients who have missed appointments or disengaged to reschedule and re-engage.
• Support rebooking workflows and collaborate with CNs and NPs to minimize patient churn.
• Perform outreach related to existing care gaps, including final MPOA follow-up post-initial appointment.
• Maintain accurate and up-to-date chart hygiene for existing patients.
• Prepare follow-up appointment documentation and organize records needed by care teams.
• Monitor Athena inboxes and respond to incoming documents and tasks that affect current patients.
• Route clinical requests and updates to the appropriate team member (SW, NP, CN).
• Respond to patient or caregiver questions related to upcoming appointments, changes, or documentation.
• Support Guide updates and alignment with ongoing care plans.
• Send follow-up appointment links and reminders.
• Manage consent follow-up workflows and help troubleshoot appointment logistics.
• Collaborate with other teams to support escalated or cross-functional needs tied to ongoing patient care.
• Participate in projects tied to fall-off reduction, Optum support, and other care continuity initiatives.
• Assist with chart organization support and documentation cleanup efforts.
