Key Responsibilities

• Assess and manage patients to identify care needs across medical, behavioral health, and specialty services.
• Partner with patients and families to establish care goals and coordinate resources to support wellness.
• Deliver primary care in collaboration with physicians and interdisciplinary teams across post-acute and long-term care settings.
• Conduct comprehensive assessments, develop care plans, and ensure timely follow-up visits (minimum standard of 260 visits per month).
• Focus on health management, education, advocacy, and preventive care.
• Maintain accurate, timely medical documentation using EMR systems.
• Review and manage medications, labs, and diagnostic tests.
• Collaborate with Care Coordinators, case managers, and PCPs to improve care quality and efficiency.
• Participate in interdisciplinary team meetings, peer review, and quality improvement initiatives.
• Provide rotational on-call coverage per company policy (evenings / weekends reimbursed).
• Ensure timely completion of medical records, wellness visits, and discharge summaries.
