RN Case Manager

A company is looking for a Clinical Guide : Case Manager, Gold Care.

Key Responsibilities

Develop long-term patient relationships and conduct holistic assessments

Create and evaluate care plans in collaboration with the interdisciplinary team and caregivers

Support chronic disease management and ensure smooth transitions of care

Required Qualifications

An unrestricted compact RN license

RN Complex Case Management experience

A minimum of 5 years of direct clinical experience in acute care

Strong knowledge of interdisciplinary teamwork

Comfort with participating in interdisciplinary discussions
