Position Summary

We are seeking a compassionate and experienced Pediatric Nurse Practitioner to join our multidisciplinary care team. This role focuses on delivering high-quality, patient-centered care to pediatric populations, emphasizing health promotion, disease prevention, and continuity of care.

Key Responsibilities

• Perform direct patient assessments, health education, and individualized care planning
• Coordinate services across disciplines and with patient insurance providers
• Advocate for patients to ensure access to medically necessary services
• Communicate with community resources to secure appropriate support for patients
• Refer patients to specialty services as needed
• Collaborate with physicians and healthcare team members to develop and implement care plans
