Registered Nurse Case Manager

A company is looking for a Case Manager RN.

Key Responsibilities

Assess member's health status and coordinate care needs, including inpatient and outpatient management

Develop and implement treatment plans, monitor health status, and adjust care plans as necessary

Identify and coordinate services and resources to provide timely and cost-effective care for members

Required Qualifications and Education

5+ years of related work experience in clinical nursing and patient assessment

Related Bachelor's degree or equivalent work experience

Graduate of an accredited school of nursing

RN - Registered Nurse Licensure and Compact State Licensure in good standing

Knowledge of medical terminology and cost containment practices
