What You'll Do
Deliver skilled physical therapy services in clients' homes
Assess mobility, strength, balance, and functional limitations
Develop and implement individualized treatment plans
Educate patients and caregivers on home exercise programs and mobility strategies
Track progress, document outcomes , and adjust care plans as needed
Collaborate with the interdisciplinary care team to ensure comprehensive, patient-centered care
What You'll Need
Active Physical Therapist (PT) license in the State of Florida
Degree from an accredited Physical Therapy program
Reliable transportation and a valid driver's license
Current CPR certification
Excellent communication and time management skills
Home health experience (preferred)
