• Setting : Free Standing Clinic
• Dates of coverage needed : ASAP - 9 / 13 with possible ongoing extension
• Schedule : Monday Friday; 8a - 5p
• Scope of Practice : Urgent Care, work related injuries, pre-employment physicals
• Experience : 2+ years of UC and Occ Med experience
• License and certification : Board Certified, WI State License, DEA, BLS, DOT Certification (must get with their training if not currently certified)
