Medical Director opportunity in Albany, NY

Wonderful Medical Director opportunity offering flexibility in your day-to-day schedule and be free of shift work. Gain a sense of professional satisfaction when you see your patient's quality of life improve and hospital re-admissions diminish. Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply : family (FM), primary care, internal (IM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).

Responsibilities :

• Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
• Represent facility in engaging, establishing, and maintaining relationships within the community
• Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
• Collaborate with leadership to review care coordination initiatives
• Develop intervention plans, and share ideas about network and risk management issues
• Work alongside pharmacists on medication management
• Implement an individualized plan of care for patients' goals
• Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
• Timely response in chart completions and pharmacy recommendations
• Provide oversight and mentorship to our clinicians in the greater Albany, New York, area

Position Highlights :

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated range of $215,000 to $242,00 annually with no cap on productivity income potential.
