Identify and assess patients’ clinical issues and health care needs.Record physical findings and formulate plans and prognosis based on patients' conditions and the treating physician's plan.Conduct evaluations and re-evaluations of established patients.Refer patients to Physicians for consultation or to specialized health resources as necessary.Collaborate with Physicians and other health professionals to create comprehensive patient care plans.Perform pain management injections in accordance with practice protocols.Prescribe and order diagnostics, therapeutics, medications, pain management procedures, and durable medical equipment as indicated.Exhibit eagerness and ability to work independently and as part of a team, demonstrating flexibility and a willingness to learn.Support the organization’s vision and culture by actively engaging in the performance improvement process.Perform other duties as assigned.Required Skills :
Proficient in using computers, especially EMR software, Microsoft Word, Excel, and Outlook.Ability to manage high-stress situations and make sound decisions under pressure.Structured, organized, and professional demeanor that fosters respect among patients and colleagues.Punctuality and great attendance are essential.Qualifications, Education, and Experience :
For APRN :
Active and valid Nurse Practitioner certificate (in the state of practice) is required.DEA Registration is preferred.For PA-C :
Valid and unrestricted professional license in Indiana, with capability to obtain clinical privileges.Physical Requirements :
This position requires the ability to sit and stand for extended periods, perform light to medium lifting, and assist in lifting patients as necessary. Reasonable accommodations will be made in accordance with ADA regulations.
