Callen-Lorde Community Health Center is dedicated to providing high-quality, sensitive, and non-judgmental healthcare to LGBTQ communities and people living with HIV. We are seeking a dedicated and compassionate Medical Provider to join our team as a Primary Care Provider (PCP). As a PCP at Callen-Lorde, you will play a vital role in delivering comprehensive care, making a positive impact on the lives of our diverse patient population. As a Primary Care Provider, your responsibilities will include :

  • Screening, diagnosing, and treating Callen-Lorde patients.
  • Providing care in a sensitive and non-judgmental manner.
  • Making appropriate internal and external referrals.
  • Offering comprehensive care based on pre-arranged schedules (20 min appts f / u and 40 min new pt appts)
  • Documenting care promptly and following charting protocols (EHR : EPIC)
  • Ensuring appropriate follow-up for ordered care, including referrals and test results.
  • Providing care to patients living with HIV. Medical providers new to HIV and other specialized care will participate in didactics, precepting, and case conferencing in order to build competency.
  • Contributing to continuous quality improvement efforts.
  • Maintaining familiarity with clinical guidelines in various areas of care.
  • Functioning effectively as part of a caregiving team.

Schedule :

Three morning / daytime shifts (8 : 30AM - 4 : 30PM, two evening shifts (12 : 00PM-8PM), and optional Saturdays (9 : 00AM-3 : 00PM)

Qualifications

  • New York State Certification / Licensure according to degree (MD, DO)
  • Current New York State Registration
  • DEA Controlled Substances Registration Certificate
  • CPR Certification
  • Experience providing primary care
  • Experience serving Callen-Lorde’s target populations preferred
  • Board certification or eligible in Family Medicine, Internal Medicine or Infectious Disease
