Key Responsibilities
Engage with members telephonically to coordinate their care and develop individualized care plans
Provide education and support for disease management and connect members with resources
Document care management activities accurately and participate in interdisciplinary team meetings
Required Qualifications
Active and unrestricted Registered Nurse (RN) licensure in Kentucky or compact licensure
Proficient in Microsoft Office Suite
Access to a private workspace for remote work
3+ years of nursing experience
2+ years of case management or home healthcare coordination experience
