A company is looking for a Patient Experience Coordinator (Remote).
Key Responsibilities
Complete individualized patient care plans and perform care management and coordination using the electronic medical record system
Maintain frequent contact with patients to provide support and manage compliance with care management programs
Document all client communications accurately and keep the Team Supervisor informed of any pertinent issues
Required Qualifications
Comfortable with speaking on the phone for extended periods
Compassionate and empathetic towards patients, demonstrating exceptional customer service
Ability to take accurate notes and multitask efficiently
Proficient in using a computer and Microsoft Office programs
Knowledge of basic healthcare terms and principles is helpful
