Your Role as a Hospice RN :
Act as the primary contact for patients and their families, guiding them through every aspect of their hospice journey.
Deliver and document skilled nursing care based on each patient's individualized Plan of Care in their home, assisted living, or wherever they reside.
Conduct initial and ongoing assessments to monitor patient conditions and adjust care as necessary.
Collaborate with a multidisciplinary team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create personalized care plans.
Provide emotional, physical, and spiritual support not just to patients but also to their loved ones, offering education, comfort, and bereavement guidance.
Teach caregivers how to safely and confidently care for their loved ones through practical training and emotional support.
Communicate essential updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
Maintain accurate, up-to-date clinical documentation and adhere to infection control and compliance standards.
Support the organization's quality improvement initiatives and scheduling efforts to ensure consistent, compassionate, and efficient care.
Assist patients and families in navigating critical topics such as :
Medication administration
Hospice philosophy and services
Symptom and pain management
End-of-life processes and expectations
What You Bring :
Active RN license in the state of employment (or eligible for obtainment).
Minimum of 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or a related nursing field.
A heartfelt approach to hospice care : compassionate, empathetic, and patient-centered.
Ability to provide care in diverse settings, including private homes and facilities.
Strong communication and critical thinking skills, especially in emotionally sensitive situations.
Valid driver's license, auto insurance, and reliable transportation.
CPR certification required.
Preferred Experience (Not Required) :
Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
Admissions RN, wound care, float pool, travel nursing
Experience with terminally ill patients or serious illness support
