Description

Responsibilities :

It is the responsibility of the RN Hospital Care Coordinator to coordinate and provide the highest level of hospice care to patients and families requiring GIP level of care in the acute care setting and those requesting to be transferred to the IPU setting for continued GIP level of care.

The RN Hospital Specialist works in conjunction with the Director of Patient Care Services, the Hospice Medical Director, hospital staff and the interdisciplinary group to :

• Assume overall responsibility for organizing patient care from an interdisciplinary group approach.
• Provide hands-on clinical nursing, interacting with patients, family, physicians, interdisciplinary group members, hospital staff, and other health care professionals involved in the patient's care.
• Assess and implement a pain / symptom management program formulated with the participation of family and patient, attending physician, hospice medical director, hospital staff and the interdisciplinary group, utilizing pharmacological and non-pharmacological methods of pain / symptom control to enhance quality of life.
• Assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues to include discussions related to withdrawal of life sustaining ventilation.
• Provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual and financial comfort.
• Participate in interdisciplinary group meetings and up-date the plan of care for assigned patients based on identified problems and needs.
• Ensure all services provided are in accordance with the patient's plan of care.
• Ensure the plan of care involves the active participation of the patient, family, PCP, hospital staff, hospice medical director and the interdisciplinary group.
• Coordinate all services related to hospice care for assigned patients in the acute care setting. Coordination of services include communicating and making referrals for spiritual counselor, social work, volunteer and ancillary services, as well as, recommendation related to the ordering of medications and durable medical equipment, patient transfers to IPU for continuum GIP level of care.
• Educate patient and family on the disease, disease process and expectations related to the prognosis, as well as expectations related to the withdrawal of life sustaining ventilation and symptom management post extubation.
• Assist with continuous quality improvement activities.
• Supervise the care and services provided by all disciplines while the patient s in the acute care setting.
• Complete all paperwork according to acute care setting policy including consents, DNR / POST, status change to 'HOS' in Cerner, admission assessment, and daily visit notes.
• Complete all paperwork according to company policy including but not limited to consents, DNR / POLST, IHC forms for withdrawal of life sustaining treatment at the in patient unit, interdisciplinary review and updates, and admission assessments.
• Complies with all nursing standards of care and standards of professional performance.
• Responsible for assigning visit frequency of all disciplines.
• Coordinate all patient transfers to IPU for GIP level of care, as requested by the patient, family, or POA. This will include the orientation and ongoing education of facility staff of IPU providing care to patients.

Admin. On Call :

The RN Hospital Specialist will be administrator on call for the palliative extubation program as part of regular job duties according to prepared schedule with DPCS. During the hours when on call, the RN Hospital Specialist is expected to follow the on call protocol and to respond to all pages within 20 minutes.

Qualifications

An active license as a Registered Nurse in good standing the state where nursing services will be performed. Graduate from an accredited school of nursing and at least one to two years experience in nursing in an acute care setting or one year of experience in home health, hospice or comparable experience is preferred. Desire to learn and a demonstration of aptitude for the hospice philosophy and comfort care required. This position reports directly to the Director of Patient Care Services.
