Responsibilities
The Nurse Practitioner is responsible for delivering annual risk assessments in residential settings or via telehealth, as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
Conduct in-home health risk assessments to identify both active and chronic disease conditions, while also evaluating all physical, mental, and social needs during the visit.Gather patient history, perform examinations, and establish diagnoses.Document patient visits in writing, adhering to NCQA standards.Obtain patient vital signs when necessary.Coordinate case management referrals and communicate with primary care providers as necessary.Engage with patients, caregivers, agency nurses, other providers, and vendors to ensure accurate diagnosis.Execute all clinical duties while following OSHA Universal Precautions.Ensure patient confidentiality is maintained at all times.Participate in mandatory meetings and in-services and contribute to committees upon request.Engage in professional development activities to maintain required licenses and affiliations.In this role, you will collaborate with :

Required Knowledge, Skills, and Experience
Active / unrestricted nurse practitioner license to practice in the required states.Board certification from one of the following : ANCC, AANP, or NCCPA.Current CPR certification.Active enrollment in Medicare / Medicaid.Valid driver's license and a good driving record required.Exceptional EHR skills.Preferred Knowledge, Skills, and Experience
Experience or training in geriatric care.Ability to work effectively in team settings and maintain strong relationships with patients, medical staff, and the public.Conditions of this role include :
Adaptability to various weather conditions and differing patient home environments.Full range of body motion, including the ability to lift and handle patients. Manual dexterity, eye-hand coordination, normal vision and hearing, standing, bending, walking, and stair climbing are essential.Regular lifting or carrying of items weighing up to 50 pounds.Able to travel in an automobile or van for up to 150 miles a day in both urban and rural settings, with the ability to drive if necessary.Pay Transparency
