Hospice Nurse (weekend Baylor position) - Work 24 hours/week and get paid for 32 hours/week!

Your Role:
As a vital member of our interdisciplinary team, you'll work under the supervision of a Clinical Manager to coordinate all aspects of patient care-ensuring individualized, compassionate support that maximizes quality of life.

Key Responsibilities:

• Conduct ongoing, holistic assessments of patients and families
• Develop and manage comprehensive care plans in coordination with the care team
• Facilitate interdisciplinary case conferences and care planning discussions
• Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support
• Advocate for patients and families, addressing physical, emotional, and psychosocial needs

What You'll Bring:

• Active Massachusetts RN license in good standing
• Associate's degree in Nursing required; BSN or equivalent preferred
• Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
• Strong interpersonal skills and the ability to thrive in a collaborative team environment
• Flexibility, self-direction, and sound clinical judgment
• Valid driver's license and access to an insured, reliable vehicle

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package - including employer-sponsored health insurance, paid time-off, tuition reimbursement, student loan paydown, supplemental insurance options and more - which adds to the company's generous total rewards package.

Pay Range

$40-$56 USD
