Position Requirement
- Under general supervision, the incumbent will plan for client services, provide basic services to individuals, and maintain records pertaining to individual progress. The incumbent must possess the knowledge, skills and abilities to fulfill the job requirements.
- A high school diploma or equivalent is required, associate degree preferred. Position also requires a valid driver license and acceptable driving record.
- Services to mentally ill and chemically dependent individuals is preferred. A flexible schedule may be required to meet the needs of the agency / programs.
DESCRIPTION :
The incumbent is responsible for assisting the case management department in providing case management services to individuals with mental illness(es) and / or substance use disorders. Emphasis will be placed on assisting the case management department with delivery of services for the SMI, SED, and ATR populations. This includes assisting the case management department with discharge planning, linking with community resources, outreach and follow-up. This position is directly responsible to the Senior Case Manager and Coordinator of Clinical Case Management Services. The position is exempt for FLSA. The position is designated as a Level Two with regard to the HIPAA Privacy Rule. OSHA regulations apply to this position.
JOB TASKS :
Responsible for helping individuals gain access to public assistance programs, searching for support groups, and locating healthier living environments.Responsible for serving as a liaison between the individual and service providers.Responsible for maintaining electronic health records per agency policies and procedures.Responsible for the provision of transportation for individuals to ensure continuity of services.Responsible for attending weekly staff meetings and participating in case discussions as a member of the clinical team.Responsible for verifying individual’s demographic information.Responsible for attending in-service training and staff development programs to enhance case management services.Responsible for other duties as assigned and consistent with job description responsible for expectations of the position.Responsible for ensuring information to outside providers is accurate and complete.Complete and mail PCP outreach and ten0day letters at the direction of the case managerAssist with scheduling of diagnostic updatesAssist with compiling documentation required to support referrals to outside providers, including medical professional and residential facilitiesProvide clerical support to case management staff as neededAssist with coordination of medication review, including ensuring individuals are aware of their appointment and assisting with transportation as needed.
