Job Description :

This Nurse Practitioner will be the first hire. Starting a program requires flexibility and patience, but it affords the opportunity to play a pivotal role in defining the initiative's clinical direction

Qualifications :

• 5+ years of independent primary care practice
• License to practice autonomously
• DEA license and comfort in prescribing for chronic diseases typically associated with ESRD, such as congested heart failure.
• Experience with dialysis or nephrology (preferred, but not required)

Preferred Qualifications :

Demonstrated expertise in the management of chronic health conditions , including but not limited to diabetes, hypertension, chronic respiratory diseases (e.g., COPD), and heart disease.
