Key Responsibilities

Manage patient care and coordinate services in a remote setting

Conduct assessments and develop care plans for patients

Collaborate with healthcare providers and stakeholders to ensure quality care delivery

Required Qualifications, Training, and Education

Active and unrestricted RN compact state license

2-3 years of clinical practice experience in a healthcare setting

2 years of experience in the healthcare and / or managed care industry

Previous experience in case management

High school diploma or GED
