Nurse Practitioner – Nursing Home / House Call (Outpatient, Part-Time Options Available)

Position Type : Full-Time or Part-Time (Flexible, including weekends only)

Work Environment : Nursing Home / House Call (Outpatient)

Location : Manhattan, NY (House Calls needed in Bronx, Nursing Home care in Manhattan)

Job Summary :

A large, well-established multispecialty group with locations throughout the Greater NYC area is seeking Nurse Practitioners to provide care in either nursing homes or through house calls . This is a permanent opportunity with flexible scheduling , strong clinical support, and the option to select the borough that works best for you.

For house calls, a car service will be provided. Spanish language skills are preferred but not required.

Essential Job Functions :

Provide comprehensive medical care in nursing home or house call settings

Evaluate, diagnose, and manage acute and chronic medical conditions

Order and interpret diagnostic tests, prescribe medications, and create treatment plans

Educate patients and caregivers on preventive care, treatment, and follow-up plans

Collaborate with physicians, APPs, and support staff to deliver high-quality care

Accurately document patient encounters in the EMR

Qualifications :

Active or eligible New York State Nurse Practitioner license

Board eligible or board certified Nurse Practitioner

Ability to practice in nursing home or house call settings

Spanish language proficiency a plus, not required
