About You In Home Care is seeking an experienced and dedicated LPN - Regional Nurse to support caregiving operations across multiple partner facilities. This role is essential in ensuring that our clients receive high-quality, safe, and compassionate care within residential, assisted living, and long-term care environments. The ideal candidate will be a licensed practical nurse with strong clinical judgment, excellent communication skills, and hands-on experience in facility-based care settings.
As a Regional LPN, you will serve as a clinical resource for caregivers and facility staff, help maintain consistent standards of care, provide training and education, and ensure all care services meet state regulations and organizational expectations. You will be responsible for reviewing care plans, monitoring quality, addressing concerns, and supporting smooth coordination between our company and the facilities we serve. This position is best suited for someone who is confident in their clinical knowledge, comfortable traveling between facilities, and able to work independently while maintaining strong professional relationships.
Your contributions will directly support caregiver performance, client wellbeing, and the overall success of our facility partnerships.
Key Responsibilities
Caregiver Support : Provide guidance, clinical insight, and hands-on assistance to caregivers working within facilities to ensure safe and compassionate care delivery.
Training & Education : Participate in caregiver training programs, assist with onboarding, and help provide ongoing education to maintain compliance and quality standards.
Quality Monitoring : Conduct in-facility evaluations to ensure care plans are being followed, documentation is complete, and care meets regulatory and company expectations.
Patient Care Oversight : Review and update facility care plans and collaborate with facility staff to ensure they align with resident needs and clinical changes.
Client & Family Communication : Communicate professionally with residents, families, and facility teams to resolve concerns, answer questions, and maintain high satisfaction.
Compliance & Documentation : Ensure accurate documentation of care activities, maintain required records, and confirm that all services follow state and facility guidelines.
Coordination with Facility Staff : Collaborate with facility management and scheduling teams to support appropriate staffing coverage and workflow efficiency.
Clinical Support : Provide hands-on clinical assistance when needed, including assessments, medication-related tasks within scope of practice, and monitoring changes in resident conditions.
Requirements
Active and valid LPN license in Louisiana
Minimum 3 years of LPN experience , including 3+ years in facility-based care , assisted living, long-term care, or similar environments
Strong knowledge of care plans, state regulations, and facility operations
Excellent communication, professionalism, and problem-solving abilities
Ability to support caregivers, handle concerns, and respond to clinical issues within facilities
Proficiency with Electronic Health Records (EHR) and healthcare software
Valid driver's license and reliable transportation for travel between facilities
Preferred Qualifications
Experience in long-term care or assisted living
Additional training or certifications in leadership, case management, or specialized nursing skills
BLS, CPR, or First Aid certification
Familiarity with WellSky (formerly ClearCare)
COVID and TB vaccinations
