Hospice LPN

POSITION SUMMARY :

The Licensed Practical Nurse - Hospice is responsible for providing direct patient care under the guidance of a Register Nurse (RN) for Overlook Hospice patients. Responsibilities include following the plan of care, providing treatments, and working collaboratively with all members of the Interdisciplinary Team to facilitate positive outcomes for Overlook's Hospice patients.

PRINCIPLE DUTIES / ESSENTIAL FUNCTIONS :

ü Provides direct patient care according to Massachusetts State guidelines for Licensed Practical Nursing.

ü Performs patient assessments and implements patient care initiated by the Registered Nurse.

ü Provides accurate and timely documentation of patient care services to reflect the patient's plan of care as initiated by the Registered Nurse.

ü Provides appropriate pain / symptom management. Evaluates and communicates patient's response to treatments and medication.

ü Participates in coordination of Overlook Hospice Services, appropriately reporting the identified needs to the Interdisciplinary Team members. Members include Registered Nurse, Physician, Nurse Practitioner, Medical Social Worker, Chaplain, Certified Nurse's Aide and Clinical Manager.

ü Provides patient and family / caregiver education and information pertinent to patient diagnosis and plan of care as initiated by the Registered Nurse.

ü Ability to use equipment and supplies effectively and efficiently.

ü Manages visit schedules efficiently, able to prioritize by patient acuity.

ü Provide support for marketing and education purposes.

QUALIFICATIONS :

• LPN, Massachusetts Licensed Practical Nurse
• Knowledge of end of life patients preferred
• Hospice or Home Care Experience preferred
• Ability to maintain confidential information
• Above average documentation / data entry skills.
• Excellent communication skills; ability to be flexible and work well with others.
• Valid driver's license and reliable vehicle is required
• COVID-19 Vaccination Required
