Job Summary :

The Hospice Chaplain provides and coordinates the delivery of spiritual and emotional support to patients, their families, caregivers, and staff.

J ob Responsibilities :

• Supports and guides patients, their families, caregivers, and staff to assist them through periods of transition, grief, loss, and anxiety.
• Provides spiritual interventions at times of crisis or death.
• Guides patients' families and caregivers through spiritual and existential questions and concerns regarding death.
• Provides comfort and solace during the end-of-life experience.
• The Chaplain must always be appreciative of and respectful toward the full range of individual beliefs, customs, traditions, and practices represented in the community, and serve as a liaison between the hospice and patient / family's community of faith.
• Performs daily visits to patients homes to address their spiritual needs.
• Coordinates care with the hospice interdisciplinary team to ensure that all of the patients and family's needs are met.
• Facilitate communication between patients, family, and the patients residential care facility to ensure all rcfe regulatory requirements are met.
• Maintains accurate documentation of patient visits.
• Available outside normal business hours for emergency response on-call.
• Performs other dues assigned.
