Nurse Practitioner - Outpatient Vascular Care

Step into a mission-driven role where your clinical judgment directly saves limbs and changes lives. As the lead Nurse Practitioner for a growing company, you will :
- Partner with interventional specialists to evaluate and treat patients with peripheral artery disease and other complex vascular conditions.
- Design and execute evidence-based care plans that restore blood flow and prevent lower-extremity amputations.
- Perform comprehensive exams, order / interpret diagnostics, and prescribe therapies, documenting each encounter in a voice-recognition-enabled EMR.
- Provide continuity of care across sites, including scheduled visits to partner nursing homes .
- Educate and empower patients and families, applying critical-thinking skills to every interaction.
- Contribute to quality-improvement and case-review initiatives that drive superior outcomes.
Requirements
Current NY State licensure and board certification as a Nurse Practitioner .Minimum 2 years' experience in a high-acuity environment such as vascular surgery, operating room, interventional radiology, critical care, trauma, or cardiology.Proven ability to craft comprehensive visit notes and autonomous care plans.Exceptional communication, patient-education, and clinical-decision skills.Wound-care and PAD experience a plus (will train the right candidate).Valid driver's license.Benefits
