Position Summary The Community Living Specialist is responsible for supporting people with intellectual and developmental disabilities in achieving outcomes aligned with their life of personal significance. This position works collaboratively with assigned staff, families, and interdisciplinary team members to ensure services are person-centered, compliant with regulatory and organizational requirements, and reflective of each individual's goals. The Community Living Specialist promotes choice, self-determination, health, safety, and community inclusion.
Responsibilities - Coordinate and contribute to the development, implementation, and monitoring of plans required by regulations, ensuring adherence to each person's Individual Support Plan (ISP) and person-centered outcomes.
- Revise and maintain ISPs to accurately reflect goals and outcomes chosen by the individual.
- Ensure the accuracy and completeness of ISP documentation and coordinate required assessments as requested by external partners.
- Respect and support each person's right to make choices and exercise self-determination, while promoting health, safety, and the dignity of risk.
- Ensure opportunities are provided for individuals to explore personal interests, participate in community activities, and develop meaningful relationships.
- Maintain ongoing communication and collaboration with families, supervisors, and team members to ensure consistent, high-quality supports.
- Perform additional duties as assigned.
- Job duties may change at any time with or without notice.
- Satisfactory Act 33 and Act 34 clearances and FBI background check.
- Valid Pennsylvania driver's license and a properly insured vehicle (mileage reimbursement provided for use of a personal vehicle).
- Ability to obtain First Aid and CPR certification within two months of hire.
- Must be flexible to work on call as needed
- Successful completion of Medication Administration training, provided by Achieva.
- Associate's Degree with four (4) years, Bachelor's Degree with two (2) years, or Master's Degree with one (1) year of experience working directly with individuals with intellectual and developmental disabilities.
