Duties and Responsibilities :
- Prepares court documents for Program Director to review at least 6 days prior to the administrative review date.
- Completes all building emergency preparedness drills per contract and COH policy requirements.
- Attends F-SPDAT quality assurance meetings for the Coordinated Assessment and Housing Placement program as scheduled. Collects and presents all relevant information for families scoring for Permanent Supportive Housing and waiting on placement.
- Completes F-SPDAT assessments for all families in program who may need permanent supportive housing support.
- Completes supply request order monthly and as needed.
- Generates and mails outreach letters and recertification letters to appropriate clients monthly.
- Prints client files from TCP, and assembles intake materials into charts for new clients, monthly.
- Completes input of HMIS data for all new intakes on a monthly basis.
- Attends trainings as required by the DC Department of Human Services.
- Attends Housing Department staff meetings and other organizational meetings as appropriate.
- Performs initial intake and assessment, and provides housing stabilization services to families.
- Networks and coordinates with community resources and makes referrals as appropriate for housing, employment, medical care, substance abuse treatment, mental health treatment, training, etc.
- Performs other duties as assigned by supervisor.
Minimum Qualifications :
Bachelor’s degree required.If applicant has a BSW / MSW degree, LSWA / LGSW or appropriate social work license required prior to start.Minimum one year work experience in social work or related field preferred.Prior experience working with individuals or families experiencing homelessness and / or living in poverty strongly preferred.Strong interpersonal, conflict-resolution, and organizational skills required.Ability to work independently and as a team required.A valid driver’s license and regular access to a car, as well as proof of auto insurance, required.At COH, we understand the toll that the Covid-19 pandemic has taken on the workforce, which is why we prioritize the following well-being and work-life balance centered benefits :
