We are currently seeking a licensed Physical Therapist (PT) to join our interdisciplinary home health team. This role offers professional autonomy, strong clinical support, and the opportunity to make a meaningful impact on patients’ mobility, independence, and quality of life.

Position Overview

As a Physical Therapist with Qualicare Home Health, you will provide skilled in-home physical therapy services , conduct comprehensive evaluations, develop individualized treatment plans, and collaborate closely with caregivers and clinical team members to deliver coordinated, outcomes-focused care.

Key Responsibilities

Conduct initial evaluations, re-evaluations, and discharge assessments in accordance with Medicare and agency standards

Develop and implement individualized treatment plans aligned with patient goals and physician orders

Provide therapeutic interventions including gait training, balance activities, strengthening, range-of-motion exercises, and fall prevention

Educate patients and caregivers on safety, home exercise programs, and proper use of assistive devices

Collaborate with PTAs, RNs, OTs, and other interdisciplinary team members to ensure continuity and quality of care

Accurately complete clinical documentation using OASIS and the agency’s EMR system in a timely manner

Monitor patient progress, identify changes in condition, and communicate effectively with the care team

Supervise Physical Therapy Assistants (PTAs) and aides in compliance with state and federal regulations

Qualifications

Active and unrestricted California Physical Therapist (PT) license

Minimum 1 year of home health experience , including OASIS documentation (required)

Experience with EMR systems; WellSky / Kinnser preferred

Strong clinical judgment, organizational, and communication skills

Ability to work independently while collaborating with an interdisciplinary team

Valid California driver’s license and reliable transportation
