About The Position

Registered Nurse (RN) - Home Health (Days) :

Description :

Registered Nurse (RN) – Home Health (Days) provides skilled nursing care to patients in their homes, ensuring continuity of care and promoting patient independence. They assess patient conditions, administer medications, and coordinate with physicians and other healthcare professionals. The RN educates patients and families on disease management and care plans. They maintain accurate clinical documentation and ensure compliance with home health regulations and standards.

Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.
