Occupational Therapist (OT) – PACE Program | Sacramento, CA

MedAdventures is seeking an experienced Occupational Therapist to join a PACE (Program of All-Inclusive Care for the Elderly) center in Sacramento, CA. In this role, you’ll support an interdisciplinary team to help participants maintain independence, safety, and overall quality of life through meaningful, function-based interventions.

    • What You’ll Do :

  • Provide individualized occupational therapy services to PACE participants
  • Develop and implement treatment plans focused on ADLs, functional mobility, and safety
  • Collaborate closely with nursing, therapy, and interdisciplinary team members
  • Educate participants and caregivers on strategies to improve daily function
  • Maintain accurate and timely clinical documentation

    • Requirements :

  • Active California OT license
  • Experience with geriatrics, home health, outpatient, or PACE preferred
  • Strong communication, critical-thinking, and teamwork skills
  • Passion for delivering compassionate, participant-centered care
