SUMMARY

Under the general direction of the Director of Family Support Services, the Director of Community Habilitation oversees all operational, administrative, and programmatic components of the Community Habilitation program. The Director ensures individuals receive high-quality, person-driven services in their homes and communities that promote empowerment, independence, and meaningful life outcomes. This role provides leadership and supervision to Community Habilitation Specialists, ensures compliance with all OPWDD regulations, supports program development, growth, and quality improvement, and provides continuous improvement efforts aligned with ADAPT’s mission.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Director of Community Habilitation include but are not limited to :

  • Leads recruitment, hiring, onboarding, scheduling, supervision, and performance management of all program staff; initiates corrective action and termination procedures when needed.
  • Provide ongoing coaching, support, and performance management to ensure high-quality service delivery.
  • Maintain availability during evenings and weekends to support staff and families during scheduled service times (no overnight sessions).
  • Conduct regular individual supervisions, team meetings, and virtual check-ins to ensure strong communication and cohesion.
  • Provide guidance and oversight to program staff, families, and participants both on-site and remotely.
  • Review and monitor service documentation, action plans, session notes, and compliance requirements.
  • Conduct routine field visits throughout the five boroughs to ensure service quality, safety, and fidelity to person-centered practices.
  • Screen, assess, and enroll new participants; collaborate with families and care teams to support successful transitions.
  • Audit program files and EMR documentation to ensure accuracy, completeness, and compliance with OPWDD and agency standards.
  • Maintain department financial records, monitor budget data, and ensure timely submission of quarterly program, billing, and utilization reports.
  • Ensure confidentiality of all participant information consistent with HIPAA and agency policies.
  • Develop, update, and enforce program policies and procedures based on regulatory requirements and best practices.
  • Build and maintain strong working relationships with care managers, families, providers, and community partners.
  • Deliver in-person and virtual presentations, conduct outreach, and promote program visibility and access.
  • Identify and connect participants to community resources that promote independence, inclusion, education, employment readiness, and overall well-being.
  • Advocate for increased access to technology, communication support, adaptive resources, and continuing education opportunities for participants and staff.
  • Manage weekly payroll processing and staff schedules.
  • Support crisis prevention and intervention using proactive communication and problem-solving strategies.
  • Foster a collaborative, respectful, and inclusive team culture aligned with ADAPT’s mission and values.
  • Perform other related duties as assigned.

Requirements : QUALIFICATIONS

  • Bachelor’s degree required; Master’s degree preferred.
  • Minimum of three years of managerial or supervisory experience in a related field.
  • At least four years of relevant experience supporting individuals with developmental disabilities.
  • Excellent written and oral communication skills.
  • Demonstrated managerial and supervisory skills.
  • Ability to read, write, and speak English.
  • Strong decision-making, critical thinking, and problem-solving abilities.
  • Strong leadership skills with the ability to motivate, coach, direct, and evaluate staff.
  • Ability to maintain professional relationships and provide constructive feedback.
  • Flexibility to work evenings and weekends based on participant and program needs.
  • Ability and willingness to travel throughout the five boroughs for home and community-based services.
  • Successful completion of all mandated in-service training and certification requirements.
  • Must pass all required background and database checks (DCJS, State Central Registry, OMIG, OIG, EPPS, SDN, etc.).
  • Passion for ADAPT's mission and demonstrates a commitment to the non-profit disability sector.
  • EDUCATION and / or EXPERIENCE

  • Bachelor's degree.
  • At least three year of managerial experience in a related area.
  • Three years of knowledge and experience in human services related to people with developmental disabilities.
  • COMPENSATION : $70,000- $75,000 Annually + Industry-Leading Benefits for all full-time employees.
