Job Summary

Our client is seeking individuals to conduct initial health assessments for a Medicaid plan. This role is primarily home-based and involves in-home work with flexibility for full-time, part-time, and varying availability including nights and weekends. Assessments may take between 15 minutes to an hour.

Key Responsibilities

• Conduct initial health assessments for Medicaid members.
• Flexible work schedule including in-home visits.
• Provide availability for day, night, and weekend work as needed.

The work is focused on the Northwest side of Phoenix and Glendale area, covering a 50-mile radius until Q1 2026, then shifting to a 50-minute driving radius.
