Description

• Assess and evaluate patients in both inpatient and outpatient situations, tailoring individualized treatment plans to meet their unique needs
• Provide physical therapy treatments focusing on enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and families through therapeutic goals, discharge planning, and home exercises
• Thoroughly document evaluations, treatments, and patient progress in compliance with facility and regulatory standards
